Split/Shared Visits

Just as you’ve gotten comfortable with Incident To billing rules, the changes made by CMS to split (shared) visits complicates things for the typical rheumatology practice. Join Jean Acevedo as she walks you through what you need to know to correctly bill under the physician or the nonphysician practitioner when they both see the patient on the same day.  Understanding the new requirements is critical to obtaining correct reimbursement.

• Split (Shared) Visits are now the place of service-specific so what to do if you bill with POS 11

• “Substantive portion” of the visit defined

• The requirements for 2022 and what they’ll be in 2023

Unique Infusion Issues with Outside Referrals

Rheumatology practices do such a good job of caring for patients receiving medication infusions that it’s not surprising more and more practices are treating patients referred by GI, ID, Dermatology, and now Neurology.   The referral is sometimes to provide the infusion and not to take over treatment of the underlying condition.  This can create compliance risks from both patient care and billing perspectives.  This NORM webinar will discuss these and provide practice recommendations for reducing these risks.

• How to overcome some of the reimbursement risks and meet incident to billing requirements.

• Attendees should be able to identify potential patient care issues and how to avoid them when infusing outside referral patients.

• Discussion on the impact of medical necessity and reporting modifier 25

Expect Surprises from the No Surprises Act and Information Blocking

The No Surprises Act is often thought of as a way to protect patients in the hospital from unexpected, high medical bills from providers, not on their plan. It is that but it’s also more. Every practice treating an out-of-network or uninsured (self-pay) patient in their office must understand the implications of this law.
One other federal regulation that needs some discussion is Information Blocking. There are some concepts and requirements you must know to be compliant when patients request their medical records.
• A thorough review of the No Surprises Act
• Discussion on the Good Faith Estimate (GFE) of expected charges for out-of-network and uninsured patients.
• Information Blocking defined and how to avoid patients reporting to the Office of Civil Rights.
• We will provide some practical suggestions for both Information Blocking and the No Surprises Act.

Medicare’s Anti-Markup Rule and 2022 Audit Trends

Practices may contract with a radiologist to read the ultrasounds, extremity MRI, and other imaging studies. From a “qualified personnel” perspective, this can be a good approach, but how do you bill? Medicare’s anti-markup rule dictates what you can charge for the professional component when paying an outside radiologist to read the study and develop the interpretation and report. Finally, we will review the audit focus we have seen in 2022.

• The anti-markup rule is not a coding issue, but it can be a compliance billing issue
• How to correctly bill when a contracted physician provides the professional component of your diagnostic services.
• We will discuss this year’s Medicare and other payer audit trends and provide some practical tips to help you “keep the money.”

  • As a speaker at the first ad hoc meeting of rheumatology practice managers gathered in a single small room at its infancy a decade ago, I’m amazed to see how NORM has blossomed into a high energy organization of depth and professional meetings with parallel break-out symposia between plenary sessions. NORM has truly come of age. This is where the “business” of rheumatology gets learned. The ”guildmanship” for rheumatology practice management is now strong.- Paul H. Caldron, DO, FACP, FACR, MBA, Arizona Arthritis and Rheumatology Associates
  • In a time of demanding changes in the management of medical practices in the US, NORM has been a lifesaver to the community of Rheumatology practices.  NORM has allowed our practice to stay ahead of the many demands of CMS and others payors and has ensured that our practice remains cognizant of new issues that arise in HIPPA compliance, human resources and medical billing to name a few. Sending our Practice Manager to NORM's conferences has been cost-effective and beneficial to our practice because she returns to our office with an abundance of information that otherwise would have taken months to compile. Every Rheumatology practice that wishes to stay on top of emerging issues in practice management should consider sending a member of their staff to NORM's conference.- Michael S. Rosen M.D., Chester County Rheumatology PC
  • Thanks to all those wonderful people in the NORM Network who respond to emails, offering their advice, experience, time, and support ... I haven't even been a member a full year yet and I am amazed at the dedication of everyone who responds to helping via emails and the NORM Organization itself! I have barely had a chance to explore the resources and I have yet to really dive into requests for help still I am silently learning so much and do occasionally offer what I can! Thank you all!- Cheryl Piambino, Kenneth E. Bresky, DO
